This is a case-based activity that includes discussion of a 23-year-old man presenting with enlarged right testis and night sweats. Ultrasound shows a 4-cm hypoechoic right testic mass with increased vascularity. An inguinal orchiectomy is performed, and pathology reveals a 4.2-cm mixed nonseminomatous tumor with lymphovascular invasion present, and rete testis involved. Post-orchiectomy tumor markers are normal by two months, and imaging reveals no evidence of metastatic disease.

At the end of this course, users should be able to:

  • Understand the AJCC staging system for testicular cancer and the workup needed to correctly stage a patient
  • Define the risk categories for clinical stage (CS) I NSGCT and know the risk of recurrence with surveillance
  • Know the management strategies for high-risk CSI NSGCT and the pros and cons of each
  • Describe the management for NSGCT that has relapsed after surveillance for stage I disease
